CommunityCare Link (CCL) is ANYF’s management services organization, blending community-based organization (CBO) expertise, cultural competence, and experience serving older adults with efficient program management and seamless contract administration. CCL is creating the contracting capacity, service quality, and IT infrastructure that allows CBOs to work more effectively with health care payers.

The NYC Department for the Aging (DFTA) launched CCL in 2017 within the Aging in New York Fund, as part of its strategic plan to better integrate aging service providers into the healthcare system. CCL benefits its member agencies, its healthcare payer customers, and most importantly, the vulnerable older adults who will gain enhanced access to needed health services.
